the regional planning process to establish an Ontario Health Team in Windsor and Essex County; and further,

THAT City Council ENDORSE a collaborative submission of the Ontario Health Team Self-Assessment Form creating one Ontario Health Team for the Windsor Essex community to the government by deadline, understanding that the governance model has not yet been determined and will continue to evolve over time; and further,

THAT City Council AUTHORIZE Administration to share information and data with the lead co-ordinating agency for the sole purpose of developing, implementing and making operational an Ontario Health Team as long as it adheres to the requirements of any funding agreements and relevant Information and Privacy Information Acts including, but not limited to, MFIPPA and PHIPA; and further,

THAT the Community Development and Health Commissioner and Executive Director of Huron Lodge, or their designates, BE AUTHORIZED to take any other steps as may be required to establish the Ontario Health Team; and further,

THAT the Chief Administrative Officer and City Clerk BE AUTHORIZED to execute any documents required to establish and participate in the Ontario Health Team, such that any funding does not create an additional annualized cost without the prior approval of City Council, and subject to legal approval by the City Solicitor, financial approval by the City Treasurer, and technical approval by the Community Development and Health Commissioner and Executive Director of Huron Lodge; and further,

THAT Administration BE DIRECTED to provide appropriate reports to City Council as the Ontario Health Team is established in Windsor and Essex County.

The Windsor Essex Ontario Health Team is comprised of over 45 local healthcare program and services providers including the three hospitals, primary care and community support providers, public health, municipalities, mental health and addictions agencies, long-term care, and other social service providers in Windsor Essex.

There are four steps to becoming a designated Ontario Health Team, a process expected to take one to three years to implement at a local level:

  1. Self-Assessing Readiness – Administration participated in the collaborative process and an Ontario Health Team Self-Assessment Form was submitted to the Ministry on May 14, 2019.

  2. Validating Provider Readiness – In July 2019, the Windsor Essex OHT was been deemed “In Development”

  3. Becoming an Ontario Health Team Candidate – Groups of providers that demonstrate through an invitational, full application process that they meet key readiness criteria are selected to begin implementation of the Ontario
